Ösophagus GIST unter 6-7 cm kann enukliert werden. Analyse an 8:8 Enukleationen vs Ösophagusresektion. po. M&M = 25% vs 50%, wobei sogar in der Ösophagusresektionsgruppe 2 Todesfälle zu beklagen waren. Medialer Durchmesser des GIST in der Ösophagusresektionsgruppe 85 mm (55–250 mm) und in der Enukleationsgruppe 40 mm (18–65 mm). Nach einem medialen follow-up von 6,4 Jahren traten 2 Rezidive in der Ösophagusresektionsgruppe vergleichen mit 0 Rezidiven ind er Enukleationsgruppe auf.
Ehrlich gesagt, mich wundert die Erfolge der Enukleation, da der GIST von den Cajal-Zellen (Vermittlungsfunktion zwischen den autonomen Nerven und den glatten Muskelzellen des Gastrointestinaltrakts, man findet sie im Auerbach-Plexus [in der Tunica muscularis zwischen der longitudinalen und zirkulären Muskulatur] und in der glatten Muskulatur der Darmwand) ausgeht und die sind in der Muskularis und nicht in der Mukosa! Wie soll dann eine Enukleation einen entsprechenden freien Resektionssaum bilden können? Nichts desto trotz scheint es bis zu einer gewissen Größe zu funktionieren und wir haben in Salzburg auch im Magen mit endoskopischer Resektion bei kleinen GISTs bis 2cm positive Erfahrungen gemacht.
